Polly Ann Weaver Smith was born in Hawthorne, California on March 11, 1963 to Betty and Gary Weaver. Not long after,  her parents moved her and her sisters to Texas where she lived permanently. In 1990, Polly married Dwayne Smith and later had two daughters. She was married to the love of her life for 31 years. Her children and grandchildren were her greatest joy and she loved watching them grow. May 7th, Polly went and joined the Lord. She will be missed by family and friends who she deeply cherished here on earth.

She is preceded in passing by her mother, Betty Weaver; her father, Gary Weaver; and her sister, Patricia Weaver.

She is survived by her husband, Dwayne Smith; two daughters, Courtney Smith and Chloe Smith; three grandkids, Ally Kitchen, Kinley Kitchen, and Weston Wiley; and her sister, Pamela Bassett.
